This is an annual, reduced-fee dental plan that allows individuals and families to receive quality dental care from Bellaire Dental Group for a percentage off the office’s usual and customary and reasonable (UCR) fees. This is not an insurance plan, no claims are filed, and no payments are given to any other healthcare providers. Bellaire Dental Group is not a licensed insurer, health maintenance organization, or other underwriter of health services. This plan may not be combined with any other offers, discounts, insurance, discount plans, or advertisements.
Membership:
Our team offers two membership options. Once the annual fee is paid, you will be entitled to the dental benefit membership program for 12 consecutive months. You have the right to cancel at any time, by submitting a written request. However, these plans are non-refundable.
Basic Membership Includes:
- (2) Healthy mouth cleanings
- (2) Exams per year
- (2) Diagnostic X-rays (for cavity detection)
- (2) Fluoride treatments
- Plus, 20% discount on all dental services
- (Excludes: Cosmetic Services, Implants, Invisalign® and Office Products)
